To the Editor: In the Video in Clinical Medicine on cricothyroidotomy, Hsiao and Pacheco-Fowler (May 29 issue)1 focus on the surgical technique. We think it is important to discuss the advantage of the Seldinger approach. In a study involving 200 cadavers, Schaumann et al. showed that the time to the first ventilation was significantly shorter (mean difference, 28 seconds) with the wire-guided Seldinger cricothyroidotomy than with the surgical technique.2 The study by Schaumann et al. also showed an increase in the risk of hemorrhage associated with the surgical method; this risk was linked to the length of the incision, which . . .
